Your servant, holding ritual rank, [name], respectfully burns incense and devoutly memorializes: the Treasure of the Tao, most true and supreme, the Three Worthies of the Most High; the Treasure of the Scriptures, most true and supreme, the thirty-six divisions of revered scriptures; the Treasure of the Masters, most true and supreme, the Great Dharma Master of the Mystery; the Great Master of the Tao of the Yellow Register, who follows the sound and answers appeals, the Heavenly Worthy Who Saves from Suffering (Taiyi Jiuku Tianzun); the Perfected Who Saves from Suffering, of great mercy and great compassion; the Perfected of Great Grace, of great mercy and great compassion; and the countless hosts of true immortals and sages who attend on the left and right in the Palace of the Eastern Pole, in the Realm of Everlasting Joy. Relying wholly on true incense, we make offerings to all alike. (State the intention.)
We humbly reflect: the Realm of Everlasting Joy of Azure Florescence is at root a heaven without sound or color; in the Palace of Wondrous Majesty of the Eastern Pole dwells the Father of great mercy and great compassion. Pervading the ten directions, he saves from suffering; through vast kalpas, he delivers people. He opens wide the gate of repentance and throws open the road of ascent. The dark realm and the bright look to him; ordinary beings and sages take refuge in him. If only one turns one's thoughts to earnest prayer, he follows the sound and answers every appeal. Now the sponsor of this fast, in memory of the departed, reverently establishes this fast and its observances. Making nine times nine prostrations, we gaze toward the dust of your passage; setting forth our sincerity point by point, we confess our faults. We humbly pray that the Most Benevolent grant our wishes, and that Supreme Virtue, which loves life, erase the black records of the Right Bureau so that the faults may be forever dissolved, and bestow the azure slips of Donghua so that the soul may swiftly ascend. Bowing our heads, we take refuge in the Great Tao of the Ultimate of Nothingness (Wuji).

The sacred realm of Jade Clarity, the heavenly palace of Pure Subtlety. Within the Yuluo Xiao Terrace, above the vast, ordered, pure depths. He manifests the Brahma form of the conditioned and possesses the divine powers of the Ultimate of Nothingness. Expounding the true mystery, he breaks the obstructions of being and nonbeing, form and emptiness; rescuing beings of every kind, he abolishes the distinctions of wise and foolish, high and low. His wondrous workings are hard to glimpse; his numinous mechanism is unfathomable. Of great compassion and great vows, great sage of great mercy, the Primordial Heavenly Worthy (Yuanshi Tianzun), who delivers people without measure.
With utmost sincerity I take refuge and pay homage. The true realm of Highest Clarity, the heavenly palace of Yuyu. Within the Jasper Terrace of Purple Tenuity, above the Jade Capital of the Mysterious Metropolis. He continues the lineage of the Primordial Void Sovereign and surpasses the roots of the Jade Kingdom of Xina. Entering the millet-grain pearl, he draws in all the pure essence of the true mystery; in the Fragrant Grove Garden, he repeatedly discourses on the means to the secret essentials. His wondrous virtue is hard to conceive; his divine powers are unfathomable. Of great compassion and great vows, great sage of great mercy, the Heavenly Worthy of Numinous Treasure (Lingbao Tianzun), who delivers people without measure.
With utmost sincerity I take refuge and pay homage. The immortal realm of Grand Clarity, the heavenly palace of Taichi. At the height of the towering Golden Gate, above the far-off ninefold empyrean. He was born through countless kalpas and preached the law in twelve thousand heavens. His five thousand secret words blend the wondrous Tao of the Three Powers; in more than eighty transformations he reached the beings of the six paths. His holy virtue is lofty; his mysterious work vast. Of great compassion and great vows, great sage of great mercy, the Heavenly Worthy of the Tao and Its Virtue (Daode Tianzun), who delivers people without measure.

Under transformation-names in the ten directions he delivers all beings; amid ten billion auspicious rays his wondrous form is beheld. He responds to appeals as occasion requires, because his vows are vast and deep. The nine-colored lotus is the far shore of the ship of compassion; a single willow sprig opens the dharma gate of sweet dew. Delivering people without measure, the greatly compassionate benevolent one. They lost the order of senior and junior, turning their backs on the duties of kinship; having gained wealth and rank, they betrayed the kindness of their teachers. They fell short in courtesy, respect, and deference, and failed to provide clothing, food, and support. Such faults and offenses are measureless and boundless. In dealing with people, they said yes with their mouths and no in their hearts; in their affairs, they were diligent at first and slack in the end. We humbly reflect: to cry out to heaven in pain is the deepest principle of the human heart; to love life and save beings is the naturalness of our Tao. If only we are pure and reverent, we will surely receive your descent. Now the jade caskets and pistil satchels have all been fully opened and proclaimed; our plain sincerity and loyal hearts have happily been brought into harmony with your response. We rejoice that the dead and the living receive grace, and trust that the dark and the bright are touched by favor.
